New Form 100 Membership Document beginning January 1, 2009


Beginning January 1, 2009, all brother Knights should begin using the new Membership Document, also referred to as “Form 100”. The Membership Document, a qualified prospect’s passport into the Knights of Columbus, has been upgraded with a new look to make it more user-friendly. This form replaces all previous versions of the Form 100 and should be used immediately. Old, unused versions of the Form 100 should be disposed of properly (recycling is the preferred method) after December 31, 2008.

The form’s most apparent change is its size, which has been increased to 8.5 x 11 inches to allow for a larger type font for easier reading, also allowing for more space for entries and to assist in determining the applicant’s interests and preferences. The document is also printed in a new maroon color.

The major revision to the Membership Document is the “New Member Annuity“, which is now available to introduce a new member and his wife to the Knights of Columbus Insurance program and to assist in planning financial stability for retirement.
